Drinking Coffee Slows Progression of Liver Disease in Chronic Hepatitis C Sufferers

(HealthNewsDigest.com) - Patients with chronic hepatitis C and advanced liver disease who drink three or more cups of coffee per day have a 53% lower risk of liver disease progression than non-coffee drinkers according to a new study led by Neal Freedman, Ph.D., MPH, from the National Cancer Institute (NCI). The study found that patients with hepatitis C-related bridging fibrosis or cirrhosis who did not respond to standard disease treatment benefited from increased coffee intake. An effect on liver disease was not observed in patients who drank black or green tea. Findings of the study appear in the November issue of Hepatology, a journal published by Wiley-Blackwell on behalf of the American Association for the Study of Liver Diseases.

Hepatitis C virus (HCV) infects approximately 2.2% of the world’s population with more than 3 million Americans infected. The Centers for Disease Control and Prevention (CDC) cites HCV as the leading cause of liver transplantation in the U.S. and accounts for 8,000 to 10,000 deaths in the country annually. Globally, the World Health Organization (WHO) estimates 3 to 4 million persons contract HCV each year with 70% becoming chronic cases that can lead to cirrhosis of the liver and liver cancer. This study included 766 participants enrolled in the Hepatitis C Antiviral Long-Term Treatment against Cirrhosis (HALT-C) trial who had hepatitis C-related bridging fibrosis or cirrhosis and failed to respond to standard treatment of the anti-viral drugs peginterferon and ribavirin. At the onset of the study, HALT-C patients were asked to report their typical frequency of coffee intake and portion size over the past year, using 9 frequency categories ranging from ‘never’ to ‘every day’ and 4 categories of portion size (1 cup, 2 cups, 3-4 cups, and 5+ cups). A similar question was asked for black and green tea intake. “This study is the first to address the association between liver disease progression related to hepatitis C and coffee intake,” stated Dr. Freedman.

Participants were seen every 3 months during the 3.8-year study period to assess clinical outcomes which included: ascites (abnormal accumulation of fluid in the abdomen), prognosis of chronic liver disease, death related to liver disease, hepatic encephalopathy (brain and nervous system damage), hepatocellular carcinoma (liver cancer), spontaneous bacterial peritonitis, variceal hemorrhage, or increase in fibrosis. Liver biopsies were also taken at 1.5 and 3.5 five years to determine the progression of liver disease.Results showed that participants who drank 3 or more cups of coffee per day had a relative risk of .47 for reaching one of the clinical outcomes. Researchers did not observe any association between tea intake and liver disease progression, though tea consumption was low in the study. “Given the large number of people affected by HCV it is important to identify modifiable risk factors associated with the progression of liver disease,” said Dr. Freedman. “Although we cannot rule out a possible role for other factors that go along with drinking coffee, results from our study suggest that patients with high coffee intake had a lower risk of disease progression.” Results from this study should not be generalized to healthier populations cautioned the authors.

National Coffee Drinking Trends 2009

In an economically challenging year, the coffee industry has proved resilient, as evidenced by new data from the NCA’s 2009 National Coffee Drinking Trends (NCDT) market-research survey. Daily consumption of coffee beverages among consumers remained consistent in 2009 with 54% of the overall adult population partaking. This is statistically on par with the 2008 figure. Additionally, cups-per-drinker and cups-per-capita, which includes drinkers and non-drinkers, continue to hold at the level from 2008 and the previous four years, but are up slightly versus 2003.

“Consumers still see coffee as an integral part of their everyday lives,” said Robert F. Nelson, President and CEO of the National Coffee Association. “Even if economic conditions cause some to alter their coffee choices, they are nonetheless continuing to enjoy coffee at levels very much on par with recent years.”

Overall Penetration
While daily penetration of coffee among American adults held steady, the data for past-week and past-year penetration of coffee are down from 2008, indicating that a small number of less frequent coffee drinkers might have moved away from the coffee category. Weekly and yearly consumption each fell by three percentage points to 63% and 77%, respectively.

Location Matters
Even as coffee retains its resiliency among consumers, coffee preparation at home is up 5 percentage points with 83% of past-day coffee drinkers reporting that they made coffee at home the day before, and 80% of past-week coffee drinkers doing so during the previous week. Out-of-home coffee preparation is down 6 percentage points for each of the same time frames.

Similar trends are seen with regard to where consumers are drinking coffee. At-home consumption rose five percentage points – now 80% of past-day coffee consumers drink coffee at home. This is the highest level since 2003. Consumers may also be taking that home-brewed coffee to go, as 10% of past-day coffee drinkers now report having coffee during their morning commute, continuing growth begun in 2007 and again matching a high set in 2006. At the same time, the number of consumers who report drinking coffee at work remained consistent at 18%, but fewer are partaking in restaurants (5%) than in 2008 (8%).

Age Breakouts
After a quick drop at the start of the recession, the pendulum steadied for young adult coffee consumers age 18-24 in 2009 with 29% now partaking in any one day; an indication that recent lifestyle changes have become ingrained enough to be maintained, even in a difficult economy. Although slightly up from 2008’s level, the current percentage of 18-24’s who drink coffee is still significantly lower than 2007’s high of 37% that was attained after several years of steady growth.

In daily consumption, coffee drinking by adults age 25+ also shows stability, continuing for the most part to maintain 2008 levels, with those in the 40-59 age group essentially on par with the 61% high set in 2007 and 2008. Modest movement continued in the 25-39 age group, returning to 2007’s 44%, which has alternated with 47% highs in 2006 and 2008.

Traditional coffee has retained its footing with consumers age 25+, showing no significant changes. Gourmet coffee consumption among older consumers has returned to levels typically seen over the past 8 years, following an exceptionally strong year in 2008.

Coffee Cheaper Antioxidant Source than Acai

Q: How many antioxidants do we need in a day? There is a great push on to buy antioxidant-rich acai berry juice, but it's expensive (I've seen it for $20 to $35). If you can't afford that, can you get the same effect from eating a good diet? - Ben, via e-mail

A: Some research shows acai (pronounced ah-sigh-EE) to be a powerful antioxidant. But North Americans get more antioxidants from coffee than from anything else in our diets. So if you're looking for convenience and don't want to shell out extra money, you don't really need to stage a hunt for that pricey little purple berry. We haven't seen any evidence suggesting that acai is a better source of antioxidants than coffee, but have seen much that says coffee is a better source than acai. (And we're not yet sure how much of an effect this antioxidant-packed berry has: We also have no data that acai increases any of your cells' three major antioxidants - SOD, glutathione, or catalase - and it's antioxidants in the cells, not the blood, that seem to be the ones that make a difference in length and quality of life.) So the You Docs' take on it is that while anything that increases antioxidant power is good, acai seems like a very expensive way of getting antioxidants.

Q: What are your opinions about whole-bone protein (known as MCHC) as a source of calcium to help keep bones strong? The kind I'm asking about is derived from New Zealand bovine (cow) bonemeal. I am concerned about antibiotics, hormones and possibly a connection to mad cow disease. Any thoughts? - Anonymous

A: Your thoughts are similar to ours. It is the bone substance that has been associated with prion, the protein particle that's at the root of mad cow disease. We think it's much better, safer and just as easy to get your calcium from a calcium citrate supplement, especially one that contains vitamin D3 and magnesium.

Q: I've been hearing talk about a bill that would remove all supplements, including vitamins, from over-the-counter availability and allow only M.D.s to prescribe them. Do you have any information about this, and do you know what can I do to fight it? - Kay, Country Club Village, Mo.

A: We know of no legislation designed to make all supplements and vitamins available only through physicians, and we don't think it's likely. But there is an existing positive vitamin and supplement regulation that's finally about to be enforced: The Food and Drug Administration is requiring supplement manufacturers to actually put into their products what the labels say they contain. This may shock you, but a surprising number of companies didn't do this. For example, in a test of CO-enzyme Q10, 30 out of 33 samples had less than 10 percent of what was promised. The tests of Hoodia were even worse. They suggest that 99 percent of the Hoodia sold in the United States has no Hoodia in the bottle whatsoever! That's why the FDA is getting stricter about regulation. Large firms must comply in 2009; small firms, by 2011.

Q: I'm wondering if you consider the supplements list in “You: Staying Young” still current or if you'd amend it now in light of new info? We follow it pretty closely; thanks! - Judy, via e-mail
A: It is up to date, with one exception: Today, we generally recommend more vitamin D3 than we did when we wrote that book, for two reasons. First, more data has been published showing that Americans are getting less vitamin D from milk and making less D in our bodies (we make D when we're exposed to sun, but we're getting much less sun). Second, there's growing evidence that extra D both fights cancer and helps keep your arteries young, and people with young arteries don't get heart disease and strokes. That's why we now suggest that you get 1,000 IU of vitamin D3 if you're under age 60, and 1,200 if you're over 60.

No higher death risk in long-term coffee drinking

WASHINGTON (Reuters) - Long-term coffee drinking does not appear to increase a person's risk of early death and may cut a person's chances of dying from heart disease, according to a study published on Monday.

Previous studies have given a mixed picture of health effects from coffee, finding a variety of benefits and some drawbacks from the popular drink. The new study looked at people who drank caffeinated or decaffeinated coffee.

Researchers led by Esther Lopez-Garcia of Universidad Autonoma de Madrid in Spain followed 84,214 U.S. women from 1980 to 2004 and 41,736 U.S. men from 1986 to 2004.

They found that regular coffee drinking -- up to six cups a day -- was not associated with increased deaths among the study's middle-aged participants. In fact, the coffee drinkers, particularly the women, experienced a small decline in death rates from heart disease.

The study found no association between coffee consumption and cancer deaths.

"Our study indicates that coffee consumption does not have a detrimental effect," Lopez-Garcia, whose research appears in the journal Annals of Internal Medicine, said in a telephone interview. "It seems like long-term coffee consumption may have some beneficial effects."

There has been a debate among scientists about the health effects of drinking coffee, which typically contains the stimulant caffeine and a number of other important compounds.

The people who took part in the research completed questionnaires on how frequently they drank coffee, other diet habits, smoking and medical conditions. The researchers then studied the mortality risk over the period of the study among people with different coffee-drinking habits.

The study found that women who reported drinking two to three cups of caffeinated coffee per day had a 25 percent lower risk of death from heart disease than women who did not drink coffee. The researchers saw a smaller decreased risk for men but it was not statistically significant.

Drinking decaffeinated coffee was associated with a small reduction in overall mortality risk, the researchers said.

The people in the study had no history of cardiovascular disease or cancer when they entered it. The women were nurses and the men doctors, dentists and other health professionals.

Some studies have indicated coffee is a great source of antioxidants, substances that may protect against the effects of molecules called free radicals that can damage cells and may play a role in heart disease, cancer and other ailments.

Recent studies have offered a mixed picture on the health effects of coffee.

A study that came out in January found that pregnant women who drink two or more cups of coffee a day had twice the risk of miscarriage as those who avoid caffeine. Another study appearing in January found that drinking caffeinated coffee lowered a woman's risk of ovarian cancer.

US coffee company to launch fortified cold drinks

A small U.S. coffee company that has latched onto the growing trend of boosting drinks with health supplements is set to launch ready-to-serve cold coffees this summer, Javalution Coffee Company has said.

The four JavaFit cold coffee drinks will have various functions including improving mental clarity, suppressing appetites, and boosting immune systems and energy levels, said Scott Pumper, Javalution president and co-founder.

"Our goal is to enhance someone's lifestyle rather than to have them make a drastic change," Pumper said.

"As health and wellness is more prevalent in this country, consumers are more educated than they have been in past years so now people are asking, 'how do I live a healthier lifestyle without making too many changes?" Pumper said.

Florida-based Javalution, an independent coffee roaster started in 2003, manufactures regular coffees, specializing in what Pumper called "functional gourmet coffees".

Functional drinks, beverages that have had supplements such as vitamins or other health properties added, are increasingly popular but are typically in soft drink or water form. Enviga, launched jointly by Coca-Cola Co (KO.N: Quote, Profile, Research) and Nestle (NESN.VX: Quote, Profile, Research) in late 2006, is a calorie-burning sparkling green tea that blends tea extracts, calcium and caffeine.

In August, Javalution will launch four ready-to-drink JavaFit cold coffees -- Diet Plus, Energy Extreme, Focus Plus Multi-Vitamin and Immune Plus Multi-Vitamin -- to be sold in U.S. outlets including convenience stores, coffee shops and health clubs.

The 11-ounce cold coffees will be packaged in Tetra Pak and were expected to sell for $2.99 to $3.29 each. The new products, an expansion of Javalution's four JavaFit coffees already sold in ground-bean blends or as brewed coffee, will expand to the European market by the end of 2008, Pumper said.

"Most people on a diet don't stay on it because they have to do something different," he said, adding that drinking coffee is already a part of many people's daily routine.

One of the many U.S. beverage trends includes consumers' growing interest in drinks that do more than please the palate and quench a thirst, said John Sicher, editor and publisher of Beverage Digest in Bedford Hills, New York.

"Many American consumers are looking for their beverages to work harder for them -- to give them an energy boost, vitamins, some nutritional value. We're seeing this across a broad spectrum of the beverage business," Sicher said.

Last year, the world's largest beverage maker Coca-Cola Co. bought Glaceau, the maker of vitaminwater -- sweet and colorful water drinks with added vitamins.

"Many consumers drink coffee for the energy function. Coffee really is a functional beverage. Beyond that, we've not seen many instances of other functional ingredients added to coffee at this point," Sicher said.

Caffeine: And Now For the Good News!

Coffeemanics, rejoice! They've been studying caffeine (again!), and this time, the news is all good. Once maligned as all-around bad for you, the lively substance has now been given a clean bill of health by ... well, everyone: the U.S. Food and Drug Administration, the U.S. Surgeon General, National Academy of Science, American Medical Association, American Academy of Family Physicians, National Cancer Institute, The Centers for Disease Control, American Cancer Society, National Research Council on Diet and Health, International Food Information Council, the Framingham Heart Study, the Association of Women's Health, Obstetric and Neonatal Nurses, and scores of researchers.

Debunking the Myths

According to recent research and endorsed by the above organizations, "moderate" intake (variously defined as three cups, six cups, and 300 mg caffeine) does not increase the risk of, or contribute to:

-any type of human cancer -cardiovascular/arterial diseases. In fact, because caffeine increases physical performance and endurance, caffeinated drinks may actually protect against heart disease. -high blood pressure. Any caf-induced rise in BP is short-lived, and generally is less than is experienced when climbing a stairway. -infertility -complications in pregnancy or adversity to fetus -bone loss/osteoperosis in adults. But here the experts hedge: They admonish persons ingesting more than three caffeinated drinks per day to offset the calcium loss with a cup of low-fat milk daily. -Caffeine does not "sober up" the un-sober.

The Performance-and-Endurance Enhancer

Athletes of all persuasions have participated in tons of tests to evaluate caffeine as an enhancer of performance and endurance, and the results, say researchers at Canada's University of Guelph, are "clear as a bell: caffeine works."

The tests show that ingesting 330 mg (or 5 mg per kilogram of body weight) 30 minutes to an hour before workouts results in longer endurance, faster times, less exertion, less fatigue, and more rapid recovery -- up to 30 percent better in each category. The most effective caffeine, though, is in tablet form, not in coffee, because the 100-plus other compounds in coffee probably block some of the effect of caffeine.

Megadoses, however, are not the right answer. Persons who do not drink caffeine steadily get the best workout-enhancer effect. In fact many groups, including the International Olympic Committee, forbid "very high doses," which they define as 600+ mg. per day (about 5 cups of drip-brewed "regular"). The other caf caveat: Don't consume while working out. As a potent diuretic, caffeine dehydrates; what exercisers take for fatigue could be serious dehydration.

The Fat Burner

None of the research reports flat-out recommends caffeine as a weight-loss/control catalyst, but the implications are pretty clear.

-Caffeine, which speeds metabolism, is the most-active ingredient in many "diet pills." -Caf breaks down fat, freeing fatty acids which are immediately burned. Conversion of fat to energy is about 30 percent more efficient when caffeine is consumed prior to exercise. Which brings up another caf caveat: The break-down, and the burning, occur only when you're in action! -While the fat is being burned, the glycogen, glucose, and amino acids (blood sugars) are being reserved -- so blood sugar levels remain higher for longer. Low blood sugar = hunger; high glucose staves starving. This is why coffee is popular among students and think-tankers. The brain functions exclusively on glucose, and higher blood sugar levels facilitate thinking.

How Much is in What?

Categorical caffeine content varies widely, as much as 50 percent plus or minus. The following are "approximate averages" in milligrams.

Caf and Kids

While all reports conclude that caffeine causes no significant loss of calcium in adults, it's an entirely different case for children. It's widely thought that coffee will, as kids say, "stump your growth." It does worse than stunt; it destroys: caffeine actually dissolves the calcium in young bones.

When a test group of 13-to-18-year-olds drank an unsweetened caffeinated drink, their urinary calcium output increased by 25 percent (to 20 mg per hour for three hours). When they drank caffeine-plus-sugar, their calcium loss was 30 mg/hour. Phosphorus, found in most colas, accelerates bone loss even more; one cola costs as much as 120 milligrams of calcium. Furthermore, a soft drink after a workout also depletes children's sodium, chloride, and potassium, causing sore muscles and delayed recovery time after exercise.

Super-caffeinated coffee creating big buzz

Tom McBride was in for a Shock recently. McBride took the plunge for the first time and added some "hyper-caffeinated" Shock coffee to his regular brew at the Gas City at Laraway Road and U.S. 52. "I can taste the difference," McBride said as he sipped the high-octane fuel on a subzero day. "It's pretty strong. I figured today I'll give it a shot and see what happens." The new beverage is popular with a wide variety of customers, said Gas City manager Colleen Olszewski. Cops, construction workers and truck drivers all want an extra jolt of caffeine in the morning, she said. Midnight shift workers hit the station about 6 p.m. looking for a Shock."It sells real well here," Olszewski said. Shock has 50 percent more caffeine than regular coffee. Frankfort-based Gas City has tested Shock in a handful of its stations in the past few months. The brew has done so well, the company has decided to roll it out to all 40 outlets in the Chicago area.Gas City isn't the only coffee seller rolling out a super-caffeinated coffee:

-Turbo: Dunkin' Donuts lets you add Turbo shots of espresso to any coffee product, hot or cold, to up your caffeine intake.
-Fusion: 7-Eleven has Fusion, an extra-caffeinated coffee mixed with energizing herbs. The convenience store also features something called Stok (pronounced stoke), a sweet "Black Coffee Shot" containing 40 milligrams of caffeine.
-Rev: Thornton's gas stations rolled out Rev, a caffeine-boosted coffee, a few days ago to energize its customers' engines. Ads warn consumers: "This coffee is obscenely caffeinated."
-High Voltage: BP Amoco's AM-PM marts are selling a range of caffeinated coffees that culminate in what they refer to as High Voltage. They also have Boost-A-Shot energy mixes consumers can pump into their coffees, too.
-Shock Wave: Speedway started selling this bodacious brew in a blue pot on Jan. 1. Ads for the coffee urge customers to "Get more kick in your coffee."

"It gets me going in the morning," Joliet Speedway manager Rich Hale said of Shock Wave. "It has a good taste. It's not real strong." Joliet BP Amoco manager Carl Mitchell said he drinks High Voltage every day."And I can still go to sleep at night."Coffee sellers are trying to tap into some of the energy drink market, said Cindy Vasquez, a Gas City area supervisor. The popularity of energy drinks laced with caffeine has other beverage producers hopping on the hopped-up beverage bandwagon."We're trying to attract young people who are looking for something hot that has a bigger caffeine boost," Vasquez said.The theory is, consumers who enjoy a caffeine-laced Red Bull or Monster might want a hot Shock in the morning, she said. The caffeine-laden pop, energy drink and coffee market is exploding right now in popularity, said John Sicher, editor and publisher of Beverage Digest in Bedford Hills, N.Y. Pepsi has Amp, he noted, and Coke has Full Throttle. "They're popular because of the energy boost consumers get," he said. "There's instant gratification." Gas City cashier Stase Elftmann said she sips a Shock around 10 a.m. each day to ward off fatigue. "That will wake you up," she said as she offered a sample to a groggy reporter.Not everyone is opting for a jolt of extra caffeine, however. Retired Will County Sheriff's Deputy Rich Walden stopped at the Laraway Road Gas City recently for a regular cup of coffee. He took a pass on Shock. With high blood pressure and a 10- 12-cup-a-day coffee habit, Walden said he plans to avoid super-caffeinated beverages for a very good reason. "My doctor told me not to (drink them)."But more and more people are reaching for the juiced java. "Everybody wants something that has caffeine or something that gives them energy," said Beth Lockhart, who manages a 7-Eleven in Shorewood. Uber caffeine cravings can mean only one thing, said Joliet Thornton's manager Judy Griffin. "Everybody is tired."

Caffeine: And Now For the Good News!

They've been studying caffeine (again!), and this time, the news is all good. Once maligned as all-around bad for you, the lively substance has now been given a clean bill of health by ... well, everyone: the U.S. Food and Drug Administration, the U.S. Surgeon General, National Academy of Science, American Medical Association, American Academy of Family Physicians, National Cancer Institute, The Centers for Disease Control, American Cancer Society, National Research Council on Diet and Health, International Food Information Council, the Framingham Heart Study, the Association of Women's Health, Obstetric and Neonatal Nurses, and scores of researchers.

Debunking the Myths

According to recent research and endorsed by the above organizations, “moderate” intake (variously defined as three cups, six cups, and 300 mg caffeine) does not increase the risk of, or contribute to:

•any type of human cancer
•cardiovascular/arterial diseases. In fact, because caffeine increases physical performance and endurance, caffeinated drinks may actually protect against heart disease.
•high blood pressure. Any caf-induced rise in BP is short-lived, and generally is less than is experienced when climbing a stairway.
•infertility
•complications in pregnancy or adversity to fetus
•bone loss/osteoperosis in adults. But here the experts hedge: They admonish persons ingesting more than three caffeinated drinks per day to offset the calcium loss with a cup of low-fat milk daily.
•Caffeine does not “sober up” the un-sober.

The Performance-and-Endurance Enhancer

Athletes of all persuasions have participated in tons of tests to evaluate caffeine as an enhancer of performance and endurance, and the results, say researchers at Canada’s University of Guelph, are “clear as a bell: caffeine works.”

The tests show that ingesting 330 mg (or 5 mg per kilogram of body weight) 30 minutes to an hour before workouts results in longer endurance, faster times, less exertion, less fatigue, and more rapid recovery -- up to 30 percent better in each category. The most effective caffeine, though, is in tablet form, not in coffee, because the 100-plus other compounds in coffee probably block some of the effect of caffeine.

Megadoses, however, are not the right answer. Persons who do not drink caffeine steadily get the best workout-enhancer effect. In fact many groups, including the International Olympic Committee, forbid “very high doses,” which they define as 600+ mg. per day (about 5 cups of drip-brewed “regular”). The other caf caveat: Don’t consume while working out. As a potent diuretic, caffeine dehydrates; what exercisers take for fatigue could be serious dehydration.

The Fat Burner

None of the research reports flat-out recommends caffeine as a weight-loss/control catalyst, but the implications are pretty clear.

•Caffeine, which speeds metabolism, is the most-active ingredient in many “diet pills.”
•Caf breaks down fat, freeing fatty acids which are immediately burned. Conversion of fat to energy is about 30 percent more efficient when caffeine is consumed prior to exercise. Which brings up another caf caveat: The break-down, and the burning, occur only when you're in action!
•While the fat is being burned, the glycogen, glucose, and amino acids (blood sugars) are being reserved -- so blood sugar levels remain higher for longer. Low blood sugar = hunger; high glucose staves starving. This is why coffee is popular among students and think-tankers. The brain functions exclusively on glucose, and higher blood sugar levels facilitate thinking.
How Much is in What?

Categorical caffeine content varies widely, as much as 50 percent plus or minus. The following are “approximate averages” in milligrams.

Caf and Kids

While all reports conclude that caffeine causes no significant loss of calcium in adults, it’s an entirely different case for children. It’s widely thought that coffee will, as kids say, “stump your growth.” It does worse than stunt; it destroys: caffeine actually dissolves the calcium in young bones.

When a test group of 13-to-18-year-olds drank an unsweetened caffeinated drink, their urinary calcium output increased by 25 percent (to 20 mg per hour for three hours). When they drank caffeine-plus-sugar, their calcium loss was 30 mg/hour. Phosphorus, found in most colas, accelerates bone loss even more; one cola costs as much as 120 milligrams of calcium. Furthermore, a soft drink after a workout also depletes children's sodium, chloride, and potassium, causing sore muscles and delayed recovery time after exercise.

Caffeine: Good For Your Skin

We all know what caffeine can do for our moods in the morning, but caffeine is also increasingly celebrated in the dermatological community for its impact on the skin.

For years, caffeine’s ability to quickly and effectively constrict blood vessels has made it a valued topical ingredient, particularly in products designed to minimize facial flushing. For that reason, caffeine can be a tremendous boon to those who suffer from rosacea, which is essentially caused by frequently dilated blood vessels that lose their ability to contract.

In fact, research suggests that both oral and topical caffeine may offer powerful anticarcinogenic benefits. A series of studies performed on mice found that caffeinated green and black teas prevented sun damage and even repaired damage once it occurred, whereas decaffeinated teas did not.

One particularly promising study suggested that topical caffeine may also repair UV damage - and that it may prove to be a stronger antioxidant than certain green tea polyphenols, currently among the strongest and best-researched antioxidants around. (On a related note, coffeeberry, which comes from the fruit of the coffee plant, is also getting a lot of attention lately as the next big antioxidant.)

Just a couple words of caution, though: Caffeine is dehydrating, so be sure to follow your coffee or tea with plenty of water. And if you’re prone to facial flushing, hot beverages can cause redness; consider enjoying your caffeinated beverages over ice.

Acute effects of ingesting JavaFit Energy Extreme functional coffee on resting energy expenditure and hemodynamic responses in coffee drinkers


Abstract

Background

The purpose of this study was to examine the effects of a functional coffee beverage containing additional caffeine, green tea extracts, niacin and garcinia cambogia to regular coffee to determine the effects on resting energy expenditure (REE) and hemodynamic variables.

Methods

Subjects included five male (26 ± 2.1 y, 97.16 ± 10.05 kg, 183.89 ± 6.60 cm) and five female (28.8 ± 5.3 y, 142.2 ± 12.6 lbs) regular coffee drinkers. Subjects fasted for 10 hours and were assessed for 1 hour prior (PRE) and 3 hours following 1.5 cups of coffee ingestion [JavaFit™ Energy Extreme (JF) ~400 mg total caffeine; Folgers (F) ~200 mg total caffeine] in a double-blind, crossover design. REE, resting heart rate (RHR), and systolic (SBP) and diastolic (DBP) blood pressure was assessed at PRE and 1, 2, and 3-hours post coffee ingestion. Data were analyzed by three-factor repeated measures ANOVA (p < 0.05).

Results

JF trial resulted in a significant main effect for REE (p < 0.01), SBP (p < 0.01), RER (p < 0.01), and VO2 (p < 0.01) compared to F, with no difference between trials on the RHR and DBP variables. A significant interaction for trial and time point (p < 0.05) was observed for the variable REE. The JF trial resulted in a significant overall mean increase in REE of 14.4% (males = 12.1%, females = 17.9%) over the observation period (p < 0.05), while the F trial produced an overall decrease in REE of 5.7%. SBP was significantly higher in the JF trial; however, there was no significant increase from PRE to 3-hours post.

Conclusion

Results from this study suggest that JavaFit™ Energy Extreme coffee is more effective than Folgers regular caffeinated coffee at increasing REE in regular coffee drinkers for up to 3 hours following ingestion without any adverse hemodynamic effects.


Background


The popularity of thermogenic supplements containing caffeine and other herbal products has increased over the past few years. In the U.S., approximately 110 million Americans drink coffee daily, while another 51 million are occasional coffee drinkers. Coffee generally contains caffeine in amounts ranging from 65–150 milligrams depending on the type of coffee and the method of preparation. Today, caffeine is appearing in many new products such as sports gels, energy drinks, and alcoholic beverages to provide a thermogenic effect in an effort to act as an ergogenic aid [1]. Even products that naturally contain caffeine are being altered to increase the amount of caffeine content. One of these products that have been introduced to the market (JavaFit™ Energy Extreme) is a new functional coffee beverage that contains 150 mg extra caffeine per serving, green tea extract, garcinia cambogia and niacin in an effort to produce a thermogenic product that could have added benefits over regular coffee. This new functional coffee is theorized to act as a thermogenic aid and possibly have implications on elevating the metabolic rate. Due to the popularity of daily coffee consumption, the rationale of altering an existing product that is used daily is sound.

Caffeine can have many effects in the body, but typically caffeine is thought of as a way to boost an individual's energy level on both a psychomotor level as well as a physiological level as an ergogenic aid in high-intensity and endurance exercise [2-5]. Caffeine has been identified as one of the very few purported ergogenic aids that has a significant effect on improving performance [1,6]. In addition, caffeine has been shown to stimulate metabolic rate in humans [7], so there could also be clinical implications for caffeine in the areas of weight loss and weight management.

Like other stimulants, caffeine has been advertised and sold as a way to stimulate energy expenditure and facilitate weight loss. Caffeine can stimulate both lipolysis and energy expenditure [8,9]. Caffeine ingestion has been studied alone and in conjunction with various herbal and vitamin products like ephedra, green tea extracts, calcium, tyrosine, chromium picolonate, capsaicin and garcinia cambogia. An earlier study on caffeine and energy expenditure indicated that a single dose of 100 mg of caffeine had a significant effect on resting metabolic rate (3–4% increase over 150 min), suggesting that caffeine can have a significant effect on energy balance at a commonly consumed dose, and possibly have positive effects in the treatment of obesity [10]. More recently, coffee ingestion (200 mg of caffeine) resulted in a 7% increase in energy expenditure for three hours following ingestion [11]. Recent research on the effects of caffeine supports the role of caffeine as a stimulus to increase total energy expenditure. A recent study found that caffeine alone increased energy expenditure by 13%, while doubling lipid turnover [8].

Several reports have examined the role of caffeine and coffee intake on a variety of diseases and health markers. It has been reported that coffee consumption has no significant effects on the risk of coronary artery disease [12,13] and may possibly even result in lower blood pressure on a chronic basis [13-15]. One safety marker that has been examined with caffeine or coffee intake is the effect on blood pressure. Despite the research supporting that chronic coffee intake could have a beneficial effect on blood pressure, research has shown that changes in systolic blood pressure are often observed following the acute ingestion of caffeine with the addition of other herbal products [16]. Many trials have shown this effect when caffeine has been combined with the herbal product ephedra [17,18]. A recent study also found increases in systolic blood pressure following acute coffee ingestion, but it should be noted that the combination of ingredients was different from the coffee blend examined in this study [16]. In addition, recent work has suggested that caffeine does have a stimulatory effect on systolic blood pressure, but the combination of other stimulants must be taken into consideration as well [19]. Thus, the stimulatory effects of caffeine on systolic blood pressure must be realized when ingesting caffeine and other stimulatory products.

Research has supported the valuable role of caffeine in metabolism and energy expenditure, and the safety of its consumption. One possible method of incorporating increased caffeine into daily consumption is to add it to daily use products (functional beverages and/or foods). The potential benefits could possibly include maintenance and possible reductions in body weight by increasing daily energy expenditure, which could have clinical implications with the prevalence of obesity in our society. This study is the first to examine the acute thermogenic responses between JavaFit™ Energy Extreme (JF) and a regular caffeinated coffee (F). Therefore, the specific purpose of this study was two-fold: 1) to assess the acute effects of ingesting a single dose of an additional caffeine-containing coffee beverage on resting energy expenditure for three hours after ingestion and 2) assess resting indices of hemodynamic function and general side effects for three hours after ingestion.

Methods

Subjects

Five physically active males (26 ± 2.1 y, 97.16 ± 10.05 kg, 183.89 ± 6.60 cm) and five females (28.8 ± 5.3 years, 64.64 ± 5.73 kg, 167.64 ± 7.37 cm) volunteered to participate in this study. Subjects were between the ages 18–35 and had to be regular coffee drinkers to participate in this study. A regular coffee drinker was designated as one that consumed at least 3 cups of coffee per week for at least one year prior to the study. Average self-reported daily caffeine intake for all subjects was 274.29 ± 53.22 milligrams per day. Subjects were not currently (or in the past 3 months) taking dietary supplements containing creatine, arginine, androstendione, thermogenics, or any other nutritional supplement. Subjects that were eligible were informed of the requirements of the study and signed an informed consent statement in compliance with the Institutional Review Board for the Protection of Human Subjects in Research guidelines of Baylor University.

Study design

This study was conducted as a randomized, double blind, crossover design that was counterbalanced for the administration of two experimental trials. Subjects were familiarized one week prior to the start of testing. During the familiarization session, subjects completed a health history questionnaire, personal information sheet and signed an informed consent form. In addition, the subjects were verbally instructed on the study design and the requirements for each testing session. Subjects reported for first testing session on a 10-hour fast and were instructed to refrain from exercise and caffeine consumption 24 hours prior to each testing session. Subjects' height was assessed using standard anthropometry and total body weight was measured using a calibrated electronic scale with a precision of ± 0.02 kg (Bridgeview, Illinois). Subjects were then instructed to lie in a supine position for 1 hour. At the end of the 1-hour period, baseline resting energy expenditure (REE), heart rate (HR) and blood pressure (SBP and DBP) were assessed. Following the baseline assessment, subjects were randomly assigned to drink 1.5 cups (354 milliliters) of either JavaFit™ Energy Extreme (JF) coffee [~400 mg of total caffeine, 600 mg green tea extracts (EGCG), <15>

REE and hemodynamic assessment

Resting energy expenditure was assessed using a Parvo Medics' TrueOne® 2400 (ParvoMedics, Sandy, UT) integrated metabolic measurement system. Metabolic carts were calibrated everyday 30 minutes prior to the beginning of testing. Resting energy expenditure was measured in a supine position and REE values were calculated during the final 10 minutes of each 1-hour session to ensure each subject had stabilized to a resting state. Resting heart rate was assessed in the supine position with wireless Polar heart rate monitors (Polar Electro Inc., Lake Success, NY) using standard procedures at the end of each observation hour. Resting blood pressure was assessed in the supine position using a mercurial sphygmomanometer using standard procedures at the end of each observation hour.

Dietary records

The subjects' diets were not standardized and subjects were asked not to change their dietary habits during the course of the study with the exception of limiting their caffeine intake 24 hours prior to each testing session. However, subjects were required to keep dietary records for the 24 hours prior to each testing session of the crossover design to evaluate their daily macronutrient intake for carbohydrate, fat, protein and total calories. The dietary records were evaluated with the Food Processor dietary assessment software program (ESHA Research Inc., Salem, OR).

Side effects assessment

The possible side effects of the two coffees were assessed with a Side Effects Questionnaire (Table 1). Variables that were addressed included: dizziness, headache(s), fast or racing heart rate, heart skipping or palpitations, shortness of breath, nervousness, blurred vision, as well as a self-report of any other unusual or adverse effects. The questionnaire assessed both the frequency of occurrence for each variable, as well as severity of these variables at the time the questionnaire.

Statistical analysis

Dependent variables were analyzed by utilizing separate three-factor [coffee (F or JF) × gender (M or F) × time point (PRE & 1, 2, 3 hours post-ingestion)] factorial analyses of variance (ANOVA) with repeated measures for each criterion variable. In addition, the within trial changes for REE were analyzed with dependent t-tests. Data obtained from the Side Effects Questionnaire were analyzed with separate ANOVA's for each respective variable. Significant between-group differences were determined involving the Neuman-Keuls Post Hoc Test. All statistical procedures were performed using SPSS 11.0 software and a probability level of < 0.05 was adopted throughout. All data are reported as means ± standard eviation.


Results


Energy expenditure measures


The REE responses between the two coffee trials indicated that there was a significantly higher elevation in REE over the 3-hour observation period during the JF trial. Significant main effects for trial (p < 0.01) and gender (p < 0.01) for the variable REE were observed indicating that the JF trial and the males had an overall higher REE. In addition, a significant interaction for trial and time point (p < 0.05) was observed for the variable REE. Dependent t-tests supported these findings indicating a significant increase in REE from PRE to 3-hours post (p < 0.01) in the JF trial, while the F trial showed a trend (p = 0.058) for a significant decrease in REE from PRE to 3-hours post. Mean values at PRE and 3-hours post for the variable REE were 1,932.83 ± 370.25 and 1,822.91 ± 323.09 kcal for the F trial and 1,858.23 ± 412.89 and 2179.75 ± 424.34 kcal for the JF trial (see Figure Figure1),1), respectively. This change in REE from PRE to 3-hours post represents an overall average increase in REE for both genders of 14.4% (males = 12.1%, females = 17.9%) in the JF trial, whereas a decrease in REE of 5.7% from the PRE to 3-hour post observation period was observed in the F trial.

In accordance with the differences observed in REE, significant main effects for trial (p < 0.01) and gender (p < 0.01) were observed for resting VO2 indicating that the JF trial and that the females had a higher resting VO2, with no significant differences for time point or interaction effects. Overall mean values at PRE and 3-hours post for the variable VO2 were 3.55 ± 0.39 and 3.35 ± 0.47 ml/kg/min for the F trial and 3.44 ± 0.36 and 3.96 ± 0.44 ml/kg/min for the JF trial (see Figure Figure2),2), respectively. These results show a 15.1% elevation in resting VO2 from PRE to 3-hours post in the JF trial, while the resting VO2 in the F trial decreased 5.6% over the 3-hour observation period.

As for the variable resting RER, a significant main effect for resting RER (p < 0.01) was observed indicating that the JF trial had an overall lower RER, with no significant differences for time point, gender or interaction effects. Overall mean values at PRE and 3-hours post for the variable RER were 0.809 ± 0.056 and 0.759 ± 0.046 for the F trial and 0.745 ± 0.039 and 0.762 ± 0.061 for the JF trial (see Figure Figure3),3), respectively. These results represent a 2.3% elevation in resting RER from PRE to 3-hours post in the JF trial, while the resting RER in the F trial decreased 6.2% over that 3-hour observation period.

Hemodynamic measures

Hemodynamic markers were observed to examine the effect of the two coffee trials. Significant main effects for trial (p < 0.01) and gender (p < 0.01) were observed for resting SBP indicating that the JF trial had an overall higher SBP and the females had a lower SBP than males, with no significant differences for time point or interaction effects. Mean values at PRE and 3-hours post for the variable SBP were 110.0 ± 7.48 and 106.8 ± 6.33 for the F trial and 112.0 ± 5.8 and 113.4 ± 6.33 for the JF trial (see Figure Figure4),4), respectively. SBP was significantly higher in the JF trial, however the ingestion of the JF coffee did not significantly increase SBP over time during the PRE to 3-hour post observation period (p = 0.61). These results show a 1.3% elevation in resting SBP from PRE to 3-hours post in the JF trial, while the resting SBP in the F trial decreased 2.9% over that 3-hour observation period.

Additional hemodynamic markers assessed in this study included HR and DBP. A significant main effect for gender (p < 0.01) was observed for resting DBP indicating that the females had a lower resting DBP than the males, with no significant differences for time point or interaction effects (see Figure Figure5).5). The results of this study indicated that there were no significant
interactions or main effects for trial, gender or time (p '>' 0.05) for resting HR (see Figure Figure6).6). These data suggest that the consumption of both F and JF have no affect on resting HR and DBP for 3-hours following ingestion.

Nutritional analysis

No significant changes in 24-hour dietary recalls were observed between subjects for macronutrient content of protein (g), carbohydrates (g), fat (g) and total caloric intake (kcal) (see Table 2), or caffeine ingestion prior to each respective trial (F or JF).

Side effects

Subjects completed a Side Effects Questionnaire following both 4-hour testing sessions to assess any potential side effects of the coffee (caffeine) ingestion (Table 1). No significant differences were observed for the frequency of occurrence or for the severity of the side effects dizziness, headache, fast or racing heart rate, heart skipping or palpitations, shortness of breath, nervousness and blurred vision. One female subject reported light-headedness at the end of the 3-hour observation period, but this was reported following both testing sessions of the crossover design, and was more likely due to the duration of the fasting condition associated with each session.

Discussion

The major findings of this study suggest that drinking coffee containing approximately 400 mg total caffeine (an additional 150 mg per serving compared to regular, caffeinated coffee), green tea extract, garcinia cambogia and niacin does have significant effects on resting energy expenditure compared to normal caffeinated coffee for 3-hours following ingestion in healthy and physically active, regular coffee drinkers. The significant difference in REE between the two trials occurred with a slight difference in SBP, while resting HR and DBP were not affected by the addition of 200 mg of caffeine over regular, caffeinated coffee, as well as the additional herbal ingredients that are present in the experimental coffee (JF).

Previous research has supported the notion that acute caffeine and/or coffee ingestion can have significant effects on REE. These previous findings have shown increases in REE over 2 to 3-hour time periods following ingestion and have utilized various combinations of caffeine combined with additional ingredients including ephedra, black tea, garcinia cambogia, citrus aurantium and chromium polynicotinate [16,20,21]. Caffeine has been the common ingredient in all of these studies and caffeine alone has been demonstrated to affect REE in human studies [7,8,10,11]. This is important because another previously studied ingredient (ephedra) that has been combined with caffeine is not overly available on the market and now other ingredients are being combined with caffeine to determine if they can alter the metabolic rate following ingestion. This study examined a functional coffee beverage that has 150 mg of additional caffeine per serving as well as an herbal combination of green tea extracts, garcinia cambogia and niacin. This is the first study to examine the acute changes in metabolic rate using this type of functional coffee. Our findings suggesting that consumption of JF can have significant effects on REE are in agreement with previous research indicating that caffeine, coffee and green tea have been shown to have positive effects on REE in humans [7,8,10,11,16,22].

Knowing the effects that caffeine and green tea extracts can have on the metabolic rate, the increases in REE observed in this study are likely a result of adding additional caffeine and green tea extracts that were not present in the regular coffee trial. In addition, the relatively small amounts of garcinia cambogia and niacin are probably not significant enough to affect any of the variables measured in this study. Garcinia cambogia has been studied as a possible means of increasing metabolism and promoting weight loss, but research has not supported these proposed benefits [23].

In the current study, the JF trial had a significantly lower RER when compared to the F trial. However, no significant changes were observed over time for RER, thus these findings suggest that fuel oxidation was not significantly affected by the experimental coffee trial. Our findings do not agree with a recent study that utilized a similar coffee blend containing additional caffeine, citrus aurantium and chromium polynicotinate which reported a significantly higher RER in the experimental coffee over placebo [16]. These differences are most likely attributed to the differences in the added ingredients to the coffee blend (i.e., green tea extracts and niacin vs. citrus aurantium and chromium polynicotinate).

The alterations in REE and RER were observed in the presence of a significantly higher resting VO2 in the JF trial. Resting VO2 increased 15.1% from PRE to 3-hour post time point, while the VO2 of the F trial decreased 5.6% over the same time period. Despite these changes, the interaction effect for trial and time point for the variable VO2 only showed a trend for significance (p = 0.06), thus we cannot suggest that the JF trial elicited a higher VO2 response over the 3-hour observation period. However, these findings in conjunction with the significant elevations in REE do suggest that ingestion during the JF trial positively affects oxidative metabolism in the body in comparison to the F trial.

An increase in SBP is an occurrence that has previously been associated with the ingestion of products containing caffeine and/or herbal ingredients [16,18,19]. The findings of this study found a significantly higher SBP in the JF trial. Despite the significant difference, the small increase over time (1.3%) was not a significant elevation. It should be noted that the JF coffee compared to the regular coffee (F) did have approximately 150 mg more caffeine per serving, and the F trial did demonstrate a decrease (2.9%) in SBP over the same time period. It is likely that the additional caffeine and possibly the green tea extracts contained in the JF coffee resulted in the small sustained increases in SBP over time. In addition to the SBP responses, the hemodynamic variables resting HR and DBP were not affected by either of the coffees, which are in agreement with previous research [16,24].

Caffeine consumption has traditionally been associated with other issues concerning health; however the claims seem to not be supported. As for blood pressure, caffeine consumption has been linked to acute increases in blood pressure, but habitual coffee consumption does not seem have adverse effects on blood pressure [15]. However, some studies do suggest that individuals that already have existing high blood pressure issues may benefit from reducing or restricting coffee intake in these populations [25,26]. Overall, habitual coffee consumption does not seem to lead to negative effects on blood pressure unless there is a pre-existing problem with high blood pressure. Diabetes is another area of health that has been studied in its relationship to caffeine/coffee consumption. Studies suggest that coffee drinking is associated with a higher insulin sensitivity and a lower risk of type 2 diabetes [27-31], and that total caffeine intake from all sources was associated with a significantly lower risk for diabetes in men and women [29]. In addition, various studies have found no negative relationship between caffeine, coffee or tea consumption with the following types of cancer: breast cancer [32], oral/pharyngeal and esophageal cancer [33], colon cancer [34], epithelial ovarian cancer [35], liver cancer [36], thyroid cancer [37] and pancreatic cancer [38] in various populations. Thus, the overall health implications with caffeine and coffee consumption do not seem to have a negative effect on health and the possible positive effects on REE and metabolism show potential in the areas of weight management and weight loss.

Conclusion

The major findings of this study indicate that the consumption of 1.5 cups of a functional coffee beverage containing approximately 400 mg caffeine (150 mg additional caffeine per serving compared to regular, caffeinated coffee), green tea extracts, garcinia cambogia and niacin can significantly increase the resting energy expenditure over a 3-hour time period in comparison to regular caffeinated coffee. These findings could have potential clinical implications in the area of weight maintenance and weight loss. However, the implications of caffeine intake on other processes in the body must not be discounted and should be taken into consideration before this is considered a possible path for improving weight maintenance and weight loss. Despite the overall difference between trials, SBP was not affected over time with the additional caffeine consumption present in the JF coffee. Resting HR and DBP were not affected with ingestion of either of the two coffees and both coffees were well tolerated with no significant reported side effects following ingestion. Future research needs to be conducted to examine the chronic effects of this product to see if it has implications in weight loss and/or weight maintenance resulting from the acute increases in resting energy expenditure.
